How we assess if you are eligible for services
To make sure we offer fair access to care services for all adults who ask for our help, we use the national eligibility criteria for care and support, which is set out by the government for all local authorities
If you do not meet the eligibility criteria for social care support we can offer a wide range of information, advice and guidance about help that is available for you, your family and carers.

Continuing Health Care (CHC)
NHS continuing healthcare is for people who may receive a 'continuing care package' if they have needs arising from disability, accident or illness that can't be met by existing universal or specialist services alone
To be eligible for NHS continuing healthcare, you must be assessed by a team of healthcare professionals (a "multi-disciplinary team") as having a "primary health need".
